#  驚！70歲洗腎翁日吃一顆番茄險喪命

70歲陳先生有糖尿病、高血壓病史，接受血液透析5年，今年2月因倦怠無力被送至急診，檢驗血鉀 9.0 meq/L、心律不整；病人自述最近數週每天吃一顆牛番茄。另一名69歲林先生有糖尿病、高血壓、冠狀動脈疾病，接受血液透析5年，今年3月因發燒被送至急診，檢驗血鉀 6.68 meq/L及心律不整；病人自述最近數周每天吃1～2斤小番茄。這二位皆經緊急血液透析及醫療後，才使病情穩定。徐碩鴻醫師表示，番茄屬高鉀食物，血液透析（俗稱洗腎）患者不宜多吃。

#### 慢性腎臟病患者避免高血鉀症

徐碩鴻表示，身體中的鉀離子在維持細胞膜電位穩定，是神經脈衝和肌肉收縮中不可或缺的成分，可調節神經、肌肉及心肌的興奮性。一般人一天可攝取4～6公克的鉀，以避免鉀離子不足。

鉀離子主要經由腎臟產生尿液而排泄，當腎功能衰竭時則降低排泄鉀離子的能力，尤其第五期慢性腎臟病患者，當體內鉀離子過多則會引起高血鉀，嚴重時身體會有不適的症狀，甚至危及生命。因此，慢性腎臟病患者必須留意飲食與藥物服用，避免發生高血鉀症。

#### 嚴重引起心室纖維顫動、心跳停止

血鉀濃度大於5.5meq/L以上即是高血鉀症，而血液透析患者常因透析不足、攝取過多高含鉀食物、藥物、便秘、腸胃道出血、血糖控制不良、代謝性酸中毒而引發高血鉀症，且會有手指麻痺、嘴唇麻木、疲倦、肌肉無力等神經症狀；或是心臟不適、心率不整、心跳變慢，嚴重時會心室纖維顫動、心跳停止。

#### 血液透析患者減少鉀的攝取

徐碩鴻醫師提醒，血液透析患者要減少鉀的攝取，不吃生菜、精力湯、麥草汁；蔬菜清燙3～5分鐘再拌油；避免飲用茶、咖啡、果汁、中藥湯汁；少喝雞精、牛肉湯、魚湯、排骨湯、火鍋湯、高湯等肉類湯汁；勿使用低鈉鹽或健康鹽；避免高鉀水果，中、低鉀水果適量選用，建議水果量一天不超過兩顆棒球的份量，也要了解服用的藥物種類。 (徐碩鴻醫師 - 台南市立醫院腎臟科主任)
