#  睡前4-6小時勿暴飲暴食 以免胃食道逆流加劇

「胃食道逆流」議題近日因社會案件而受到重視。消化內科主治醫師黃世斌表示，一般食物需要4到6小時才會充分消化，胃食道逆流患者最好等食物充分消化再就寢，避免睡前4到6小時暴飲暴食，大吃大喝後也要避免馬上躺平，以免胃食道逆流惡化。

**胃酸逆流 導致食道發炎**

黃世斌指出，胃食道逆流代表胃酸逆流，胃部有很強的保護機制，可以減少胃酸傷害，而食道就沒有這種防護機制，只要遇到胃酸，很容易就會受到傷害，只能靠食道與胃之間的賁門緊密，阻止胃酸逆流，當一個人吃東西吃太快、吃太多，胃賁門就會鬆弛，導致胃酸逆流至食道，引起食道發炎。

**多數改變習慣後獲改善 用藥或手術是進一步療法**

胃食道逆流症狀包括火燒心、胃酸逆流，也有一些患者出現胸悶、咳嗽、喉嚨有異物感等非典型症狀。多數患者改變生活、飲食習慣後，症狀就會獲得改善；但也有少部分患者用藥效果不佳、症狀反覆發作，此時就要考慮接受手術治療。黃世斌醫師解釋，手術治療是在幫鬆弛的賁門關緊，但這終究是侵入性手術，還是有些治療風險。

**月餅及柚子吃太多、吃宵夜 當心胃酸逆流加重**

到了中秋節連假，很多民眾會吃月餅、柚子等食物，可能還會喝點含糖飲料。黃世斌醫師提醒，如果吃太多月餅、柚子，胃酸分泌會增多，若熬夜、晚餐時間過晚、睡覺前吃宵夜，胃食道逆流症狀就會更惡化。此外，像咖啡、茶等食物也會刺激胃酸分泌，因此建議胃食道逆流患者不要吃太多月餅、柚子，也要避免飲用含糖飲料、咖啡、茶等飲品。睡覺時，枕頭可以墊高一點，以緩和胃酸逆流的不適症狀。

**當心這些異狀 應就醫檢查**

黃世斌醫師也叮嚀，長期胃食道逆流患者一旦出現體重下降、吞嚥困難、吐血、解黑便等症狀，就要懷疑可能有食道癌，最好到醫院接受進一步檢查。
