#  醫說：腹部手術後這樣吃保健康

農曆春節假期是親友團聚的時刻，難免大吃大喝，所以年節期間及年節後出現了許多忽略醫師叮嚀而報到的暴飲暴食併發症患者。

**腹腔內手術容易造成胃腸道沾黏**

黃俊傑醫師, 台大醫院新竹分院外科部提醒，曾接受過腹腔手術患者更需節制飲食，細嚼慢嚥；一般而言，凡是接受過腹腔內手術的人，就有可能造成胃腸道的沾黏，容易因某些難以消化或未消化的食物卡在部分沾黏的腸道造成阻塞。

腹部手術後的患者吃東西不管任何時候都得記得要「細嚼慢嚥」，纖維較粗或是較難消化的食物像是柿子、牛蒡、香菇、糯米等，更應該咬到爛才吞嚥，容易導致脹氣的食物例如豆類、洋蔥、大蒜等，則不要吃太多，至於辛辣和油炸物品更應該避免。

**胃部手術後宜少量多餐**

而接受過胃部手術的病患，由於胃不會再生，手術後胃容量必定會減少，胃攪拌磨碎食物的能力也會降低，因此除了細嚼慢嚥之外，更記得要「少量多餐」，千萬不要吃到把胃都撐滿了而導致胃麻痺或是胃食道逆流惡化。剛接受過膽囊切除的患者，則要記得不要吃太油膩的東西，以免造成脂肪便增加而拉肚子。

黃俊傑強調，現代人由於工作忙碌，飲食習慣改變，或多或少可能有慢性胃炎、胃食道逆流或是膽結石的存在，只是因為症狀不明顯而容易被忽略，如果持續暴飲暴食或是吃下過度刺激的食物之後，都有可能造成病況惡化而引起急性胃炎、胃潰瘍、逆流性食道炎、膽絞痛甚至是急性膽囊炎、膽管炎或膽石性胰臟炎。

黃俊傑提醒民眾一定要避免暴飲暴食，腹部手術後患者要細嚼慢嚥，胃腸道切除後還要少量多餐，至於有膽結石的患者，若出現餐後上腹或右上腹疼痛的情況，也請及早就醫評估是否須將膽囊切除以避免更嚴重的併發症出現。
